8.5?11, easy to modify, with two image placeholders (smart object layers called “img holder_right click_replaceContents”). Click on the left thumbnail (right one is the mask, don’t touch), and choose Edit Contents, paste your image in the new window, resize to fit vertically, save, close. It will automatically update in the main PSD file. There are three color schemes, just enable the on you like – Top layers called Blue, Purple, Yellow (disable other two by clicking the eye icon so it’s gone).
